Check your cache settings in Admin Console, and verify their integrity.

Also, verify the anonymous IIS user account, and check Windows users permissions to the image location, as well as full control of the cache location.

Finally, verify that the image you are seeking is not corrupt by opening it in a 3^rd party imaging application (i.e. Windows Imaging).
